How to make transition from heavy to smooth-high vocal?

Like when I'm singing rolling in the deep at the line of "the scars of your love they leave me breathless I can't help feeling" <-- chest voice

then when I belt out the chorus "we could have had it "ALL~~~~~" sounds very chesty. I can belt with fully head voice but before this chorus I sang wirh my chest voice and when i belt i belt it very chesty? it simply means that I can't make that transition.
